Insurance & Billing Information

As a courtesy, we will submit claims to your primary health insurance and any secondary coverage you may have.
Before your appointment, we will make every effort to verify your insurance benefits and any pre-authorizations, and to discuss them with you. Our goal is to help you better understand your coverage and any financial obligation you may have.
Please understand that ultimately, you are responsible for payment of your account, including any balances not covered by your insurance. At the time of your visit, you are responsible for payment of any known deductibles, co-insurance or co-payments, as well as any non-covered service. We recognize the burden this may place on you or your family and would be glad to discuss a payment plan.

For your convenience, you may make payment to Lehigh Magnetic Imaging Center in a variety of ways. We accept cash, check, money orders, debit cards and major credit cards.
Our online bill payment system is a secure and convenient way to pay your bills from anywhere, at any time.
Pay Your Bill By Phone
A Billing Representative will be glad to assist you in making payment by credit or debit card. We will charge your card only for the amount you authorize. A payment plan may also be arranged. Simply call our Billing Office at 610.740.0400.
Ensure a Smooth Billing Process
Lehigh Magnetic Imaging Center is committed to making billing as patient-friendly as possible. Here are some ways you can help us:
- Please provide complete health insurance information.
At the time of your visit, you will be asked to provide proof of insurance, including any secondary coverage, information on the subscriber if other than the patient (name, date of birth, and social security number), a prescription or order and photo identification.
- Please understand and follow the requirements of your insurance plan.
Be sure to know your benefits and complete a coordination of benefits form if requested by your insurer. If you have scheduled an appointment yourself, please notify your physician as your study may need pre-authorization. Your plan may include deductibles, co-insurance or co-payments, or may not cover some services. Lehigh Magnetic Imaging Center is responsible for collecting any patient responsibility. Please be prepared to make payment at the time of your visit if your insurance plan has any out-of-pocket expenses.
- Please respond promptly to any requests from your insurance plan.
Health insurance benefits are provided under a contract between you and your insurance company. Lehigh Magnetic Imaging Center is a third party to those benefits and may need your help with your insurance. Please respond to any queries you may receive from your insurance company. If your insurance plan has not paid us after a reasonable time (usually 60 days after billing) and has not responded to our attempts to resolve payment on your behalf, the amount owed may become your responsibility.
- Please make timely payments if you have a balance due.
You will receive a Statement of Physician Services from us for amounts not paid by your insurance and for which you are responsible. It is important to remit the "Pay This Amount" as noted on your statement. Please do not wait for your insurer to pay pending balances.
